Please note, our pay-as-you-go repeat prescription fees are £25 and free if you are a subscription member.

Since your last dispensed medication have you been diagnosed with a new medical condition by your GP, a hospital specialist, or another healthcare professional?
Since your last dispensed medication have you been prescribed any new medication by your GP, a hospital specialist, or another healthcare professional?
Since your last received prescription have you had any tests (scans, x-rays, blood tests) for any condition and if so were any of the results abnormal?
Has your primary condition, for which you were prescribed your last medication, worsened at all?
